GR845 Elite Basketball Camp – Registration & Waiver

RELEASE OF LIABILITY AGREEMENT

 

I understand that participation in basketball activities including training drills, scrimmages, conditioning, and related athletic events at the GR845 Elite Basketball Camp involves inherent risks including but not limited to:

  • Sprains
  • Strains
  • Fractures
  • Concussions
  • Heat exhaustion
  • Serious bodily injury
  • Permanent disability
  • Death

 

I voluntarily allow my child to participate in the GR845 Elite Basketball Camp with full knowledge and acceptance of these risks.

In consideration for participation in the GR845 Elite Basketball Camp, I, on behalf of myself and my child, hereby release, waive, discharge, and hold harmless:

  • The GR845 Elite Basketball Camp
  • All coaches and staff
  • Volunteers
  • Facility owners and operators
  • Sponsors and affiliates

 

from any and all claims, damages, liabilities, or causes of action arising from participation in the camp, including claims based on negligence to the fullest extent permitted under New York law.

I agree that neither I nor my child will file a lawsuit or pursue legal action against the GR845 Elite Basketball Camp or its representatives for injuries sustained during participation.

 MEDICAL AUTHORIZATION & INSURANCE ACKNOWLEDGMENT

 

I certify that my child is physically able to participate in athletic activities.

In the event of injury or medical emergency, I authorize staff of the GR845 Elite Basketball Camp to administer first aid, seek medical treatment, and arrange transportation if necessary.

I understand that I am financially responsible for any medical expenses incurred.

I further understand that the GR845 Elite Basketball Camp does not provide medical insurance coverage for participants during the duration of the camp, and that I am responsible for maintaining active health insurance coverage for my child.

Participants are required to promptly report any injury, pain, illness, or physical concern to a coach or staff member of the GR845 Elite Basketball Camp.

If a participant fails to notify camp staff of an injury, discomfort, or medical concern and continues participation, the GR845 Elite Basketball Camp shall not be held liable for any resulting aggravation, complication, or future injury that may occur.

In the event a participant sustains a fall, sprain, strain, impact, or other injury deemed reasonably manageable and elects to continue participating in camp activities, such continued participation shall be considered voluntary and at their own risk.

The parent or legal guardian acknowledges and agrees that the GR845 Elite Basketball Camp, its coaches, staff, and affiliates shall not be liable for any worsening of an injury or subsequent injury resulting from continued participation after an injury has occurred.

PHOTO & MEDIA RELEASE

 

I grant permission for the GR845 Elite Basketball Camp to use photographs or video recordings of my child for promotional, marketing, and social media purposes without compensation, for the sole purpose of exposure and expanding the camp.

CODE OF CONDUCT, PROPERTY RESPONSIBILITY & INDEMNIFICATION

 

Camp participants are expected to:

  • Show respect to coaches and other campers
  • Follow all safety instructions
  • Refrain from fighting, bullying, or unsafe behavior

Participants must treat all camp facilities and venue property with care.

The parent or legal guardian agrees they are financially responsible for any damage to the building, gymnasium, equipment, or camp venue property caused by them or their child, whether accidental, negligent, or intentional.

The GR845 Elite Basketball Camp shall not be liable for property damage caused by participants.

The GR845 Elite Basketball Camp staff reserves the right to dismiss any participant for excessive misconduct, violation of camp policies, or behavior deemed unsafe or disruptive, without refund.

If a participant chooses not to continue attending the camp, or if a parent or legal guardian elects to remove their child from the camp for any reason or for any portion of the camp duration, no refund, credit, or prorated reimbursement shall be provided.

The parent or legal guardian agrees to indemnify, defend, and hold harmless the GR845 Elite Basketball Camp, its coaches, staff, volunteers, facility partners, sponsors, and affiliates from any and all claims, liabilities, damages, costs, or legal expenses arising from participation in the camp or from actions taken by the participant.

SPECTATOR POLICY

 

Parents or legal guardians of registered participants are welcome to observe camp sessions.

All parents in attendance are expected to conduct themselves in a respectful and professional manner and uphold the highest standard of sportsmanship while present at the facility. Disruptive behavior, interference with camp instruction, inappropriate language, or conduct deemed detrimental to the camp environment may result in removal from the facility.

Attendance is limited to parents or legal guardians of registered participants only.
